 home, in hospitals, long-term care facilities, and retirement homes. 

  Here is our process in setting up an In-home care assessment:  

1. Discuss care options and answer questions over the phone.

2. Arrange an in-home consultation with an Intake Care Manager to address further questions.

3. Gather health information and details about routines and preferences during the consultation.

4. Create a client-centered care plan.

5. Develop a flexible care schedule and assign a caregiver based on care needs.

6. Regularly seek feedback from both caregiver and client to adjust the care plan as needed.
